| I know this is really a mobility area, but here is the form you need to fill out to get call-a-bus for Oswego, Onondaga and Cayuga
counties, in central New York State. Click HERE to see the form. The form is 7 pages long(598 k),
so if you getting it online, it might take a minute to download, if your connection is a slow one. I suggest you save it to
your hard drive (by right-clicking and saving as) to speed things up when you want to use it. The
first two pages are instructions as to how to fill out the form, and pages 3-7 are the actual form.. When you do want to use it, just fill in pages 3-7, and then fax it to the fax number available in the directions, or snail-mail it to the address included there as well. IMPORTANT: Be sure that you sign, date, and give your relationship to the consumer, at the bottom of (with pen) pages 4 and 7. It is also important to remember, that you cannot save the information you enter in this form, so you must print it before you close the document, or that information will be lost. Note: At the bottom of page 3 on the form, a line says "My disability is (circle one) permanent / Temporary." You will notice a blue circle around Permanent. If your client's disability is temporary, you can drag that circle over Temporary, by moving your cursor around on the circle until a little black arrow appears.
